Evaluation of the shetty test to exclude fractures in patients with ankle injury in the emergency department
Aims: Ankle sprains are a leading cause of emergency department (ED) admissions. This study aimed to compare the diagnostic performance of the Shetty Test (ST) and Ottawa Ankle Rules (OAR) in excluding fractures among patients with traumatic ankle injuries.
Methods: This prospective study included 216 patients presenting to the ED. The sensitivity, specificity, predictive values, and accuracy of both ST and OAR were calculated and compared against radiographic findings.
Results: Fractures were detected in 55 (25%) patients. For the ST, sensitivity was 96.4%, specificity 61.5%, and accuracy 70.37%. For the OAR, sensitivity was 85.5%, specificity 16.1%, and accuracy 33.80%. ROC analysis showed a significantly higher Area Under the Curve (AUC) for ST (0.794) compared to OAR (0.638) (p < 0.001).
Conclusion: The ST demonstrated superior sensitivity and specificity compared to OAR, suggesting it is a valid and efficient method for determining the necessity of X-rays in ankle injuries.
